We are pleased that you have chosen us for your orthopaedic care and we are committed to helping you find the best approach, treatment and solution. To make your first visit as helpful as possible, please read through the following information about what to do prior to your appointment and what to bring with you to your first appointment.
What to Bring
It is important for us to have these items on your first visit in order to have a complete medical record to better serve you:

- Insurance card(s)
- Picture ID
- Insurance referrals
- Related x-rays, MRI’s, CT scans, etc
- Lab reports
- Medical records relevant to the visit
- List of current medications
- Payment
Please note that payment is due at the time of visit – this includes co-pays, specialist co-pays, and deductibles, and must be paid prior to your appointment. If the payment is not collected at the time of visit, the patient is still responsible for any payments due.
Arrival
In order for the provider to see you at your scheduled appointment time, please arrive at the office twenty minutes early for your first appointment so that we may process your new patient information in our computer system prior to you being seen.
To expedite this process, you may print off Patient Information Form(s) and Medical Information Form(s) in advance. Please complete it prior to your appointment. Be sure to bring it with you on the day of your appointment.
In-Office Wi-Fi
For your convenience, we offer free Wi-Fi for our patients to enjoy while in our office. If you need assistance, please ask our office staff for help.
